Both minimally invasive and maximally invasive procedures are included, but the reader must realize that maximally invasive surgery can be done through small incisions. Thus, whaen access incisions are small but the operation on the inside as good as it gets, the term "minimal access surgery" is preferred. Improved technology makes minimal access surgery availabe to more patients today. Nowhere is this less evident than in gynecology. Though laparoscopy has been a major diagnostic operation in gynecology for over 30 years, it has not yet achieved mainstream status for major gynecologic surgery. Perhaps, some of teh articles in this edition will hasten the demise of large abdominal incisions for most major gynecologic procedures. Improved technology should make it possible to optimize surgical performance by considering the physical capabilities of the surgeon in relation to the procedure, the endoscopic instruments, and the operating room environment. However, making surgical procedures foolproof for surgeons with limited ability could prove cost prohibitive. We must continue to attempt to identify the fifted surgeon and be wary of possible malfunctions by the machines we use. We hope this volume of Surgical Technology International will stimulate increased interest in the new technologies available for mutidisciplinary surgical treatment today. Technology has transformed the face of surgical technique among the disciplines of surgery. Significant pressure has been placed on both industry and medicine to develop, test, and implement devices and procedures at a rapid pace. Unfortunately, this pace has at times surpassed the system's ability to train and prepare a corps of surgeons competent in both the technical and cognitive aspects of minimal acces surgery.
